FAYETTE COUNTY HIGH SCHOOL BAND

DISCIPLINARY AGREEMENT

Since the Fayette County High School Band Program is approved and is a function of the Fayette County Board of Education, all students shall conduct themselves in accordance with the policies of Fayette County High School and the Fayette County Board of Education. The band directors will deal with minor instances of inappropriate behavior. Certain major violations of policies will result in the student(s) in question being sent home as soon as possible at their parents' expense. These violations include, but are not limited to, the following:

• Possession and/or use of unauthorized medication, drugs, alcohol, tobacco, or any other substance in violation of Fayette County Board of Education policies.
• Any student out of his/her room after curfew.
• Unauthorized departure by an individual from a group setting.
• Deliberate destruction, vandalism, or theft of property.

I, __________________________________, understand and will abide by the rules and policies governing the Fayette County High School Marching Band Program. I also understand and agree that I am subject to the consequences stated above if I violate any of these regulations.

                       PROMISE OF INDEMNIFICATION

            In consideration of permitting my child to accompany the Fayette County High School Marching Band to band camp and any and all away trips for the 2003-2004 school year, I hereby agree to indemnify and hold harmless the Fayette County High School Band, Fayette County High School, Fayette County Schools, its teachers, employees, and trip sponsors against any claim for damages, compensation or otherwise on the part of said minor(s) or his heirs, executors, or administrators and to reimburse or make good any loss or damages or costs that the Fayette County Band, Fayette County High School, Fayette County Schools, its teachers, employees and trip sponsors may have to pay if any litigation arises on behalf of any claims made by said minor(s) or anyone on his behalf as a result of injuries sustained my child(ren) on the said trips.
